Under the general direction of the Director, is responsible to the patient to ensure that each patient receives the right drug, dose, and route, at the correct time in a cost effective manner. Duties and tasks must be carried out in accordance with hospital and department missions and philosophies, federal and state laws, and professional standards and ethics.
1. Communications: (A) Interact with staff and patients and the public in a courteous and respectful manner. (B) Use appropriate verbal and written skills, documentation skills according to departmental organizational practices, policies and procedures.
2. Team Work: (A) Collaborate with peers, staff, fellow team members and outside colleagues. (B) Assist with training personnel as assigned. (C) Rotate in other job tasks as necessary. (D) Exhibit an approachable, interactive style in relationships. (E) Communicate effectively by being receptive to and receiving feedback.
3. Rules and Regulations: (A) Adheres to health system and departmental policies and procedures. (B) Complies with all requirements related to risk management, safety, security, fire, and infection control. (C) Complies with all applicable federal, state, and local laws, rules, and regulations.
4. Professional Skills - Hospital: Ensures safe, quality, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols by: (A) Interpreting medication orders (verbal and written) and verifying the orders for accurate patient medication profiles. (B) Preparing and dispensing medication orders accurately via drug distribution systems. (C) Compounding and dispensing pharmaceuticals, including sterilely prepared chemotherapy and parenteral nutrition products. (D) Issuing controlled substances to patient care areas and maintaining records as required by law. (E) Managing pharmacokinetic regimens. (F) Evaluation patient profiles for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose for patient age and disease state(s). (G) Assists with TPN consult service. (H) Conducting medication reconciliation for inpatients during their stay. (I) Review/interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as appropriate. (J) Read extracts and interprets information in EHR accurately and responds appropriately. (K) Documents all clinical activities and intervention accurately and completely. (L) Provides clinical consultation and clarification to practitioners. Suggest appropriate, cost-effective therapeutic alternatives to medical staff as appropriate. (M) Sustain the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection. (N) Attends patient care rounds on their units. (O) Provides accurate, adequate, and timely drug information to the professional staff and to patients and their families as needed. (P) Organizes and prioritizes individual work assignments.
5. Professional Skills – Clinic: (A) Work under the approved protocols to educate and monitor patients on anticoagulation therapy. (B) Maintain documentation and communication to keep the referring physician informed of their patient’s status. (C) Consult with the referring or supervising physician regarding problems or complications that require medical decisions. (D) Remains current with anticoagulation literature and suggests changes to the Director of Pharmacy to improve service. (E) Provide patient education and communications at level patient understands, so that they may assume an active role in preventing anticoagulation related complications. (F) Provide anticoagulation and other drug information as requested by patients, providers and other healthcare professionals. (G) Maintain accurate clinical and billing documentation. (H) Maintain data to monitor outcomes. (I) Provide “bridge therapy” for patients undergoing procedures or surgeries. (J) Provide education, therapy and follow up for patients who can be treated for deep vein thrombosis on an outpatient basis. (K) Oversee the training of Anticoagulation Clinic staff and serve as a resource for anticoagulation clinic staff.
6. Quality: (A) Participates in the quality improvement and medication use review activities of the department. (B) Collects data; conducts quality monitors and inspections; and maintains logs, records, and other documentation as assigned. (C) Detects and reports suspected adverse drug reactions accurately and in a timely manner.
7. Supervision: (A) Provides direction, oversight and supervision for students, residents, pharmacy technicians or assigned staff. (B) Works independently with minimal supervision. (C) Organizes and prioritizes work assignments for assigned staff. (D) Ensures pharmacy services are provided in a timely manner.
8. Training and Education: (A) Participates in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staffs. (B) Attends orientation, education, and training programs pertinent to the delivery of patient care on their teams. (C) Reviews literature and other materials pertinent to the practice of pharmacy. (D) Attends in-services and staff meetings on a regular basis.
9. Other Duties: Performs other duties as requested.
