Position Summary

  • Development of Diabetes (DM) program infrastructure.
  • Monitoring of patients admitted to Vidant Medical Center with diabetes and hyperglycemia; medication/disease state management for Diabetes Program; compliance with DM performance measures and quality metrics across heterogeneous populations.
  • Provision of drug information and glycemic management consults.
  • Human Resource Management - Plans, selects, develops, and evaluates program staff.
  • Fiscal Responsibility- Contributes to the facilitation and maintenance of a cost effective care delivery system for target DM population.
  • Serves as a liaison between DM Program and Pharmacy, Nursing, Nutrition, physicians and extenders on DM Program related issues.
  • Oversight, assessment and provision of diabetes self-management education for patients and families admitted to Vidant Medical Center with primary or secondary diagnosis of diabetes.
  • Education of health care professionals on pharmacotherapeutic topics relating to DM.
  • Collaborate with Medical Director and Electronic Health Record orders development team to assure DM order sets and protocols reflect best practice.
  • Prepares Vidant Medical Center for JC Advanced Disease-Specific Care Certification for Inpatient Diabetes Care.
  • Serves as a content expert on DM-related issues for Vidant Health community hospitals.
  • Serves as an information resource for the re-evaluation and implementation of therapies for the in-hospital management of diabetes.
  • Works with physician and nursing staff members in the development and use of investigational or research protocols for DM population.
  • Functions as primary resource for IV insulin management and transition from IV to SQ insulin.
  • Performs other duties as assigned.

Minimum Requirements

  • Doctorate in Pharmacy
  • Current, or within 2 years of hire, Certified Diabetes Educator (NCBDE), or Board Certified in Advanced Diabetes Management (AADE).
  • 3 years experiencerequired (residency training or equivalent clinical experience).
  • More than 2 years of experience in practice area may substitute for residency training.
