Job Description
SCOPE OF POSITION
The Pharmacist, Clinical Specialist Medication Therapy Management (MTM) is responsible for providing personalized and patient centered MTM services for the purpose of optimizing medication therapy to improve clinical, economic, and humanistic outcomes. The MTM Clinical Pharmacy Specialist participates in diverse services including comprehensive medication reviews, targeted interventions, adherence monitoring interventions, cost-effective interventions, transitions in care, and post-discharge medication reconciliation to achieve these outcomes. The pharmacist may be responsible for patient education on proper medication use, potential side effects, and lifestyle modifications. The team member will be responsible for monitoring patient's response to medication therapy and making necessary recommendations to patients and providers. The pharmacist will maintain accurate documentation of all work-related activities. Consultations will be completed in person or via telephonic or virtual platforms.
The pharmacist will coordinate and collaborate with cross functional teams throughout the health system and community. The team member will practice under the direction of leadership and collaborating physicians to manage referred patients' medication therapy.
An ideal candidate must possess broad pharmacotherapy knowledge for understanding patient medication therapy needs. Excellent oral communication skills with patients, providers, and team members, plus demonstrated customer service skills are necessary. Attention to detail with good critical thinking and problem-solving skills are required. The pharmacist will serve as a mentor and preceptor for students.
The Clinical Pharmacy Specialist plays a pivotal role in establishing and optimizing departmental operations. This includes spearheading initiatives to identify and address information technology requirements, ensure compliance with regulatory standards, and cater to the needs of various payors. Additionally, they are tasked with crafting and assessing critical reports and metrics to gauge the effectiveness and efficiency of pharmacy services.

EDUCATION AND EXPERIENCE
Must be a graduate of an ACPE accredited college of pharmacy with an advanced pharmacy degree (e.g., Pharm.D).
Minimum of 3 - 5 years of relevant experience serving as a pharmacist in an ambulatory or community pharmacy practice setting, strong knowledge of MTM billing, experience of medication transitions of care preferred.
Completion of a PGY 1 Pharmacy Practice residency and PGY 2 residency in applicable specialty is required.
Must hold or be or be eligible to hold a valid and active pharmacy licensure in both Tennessee and Virginia
Board Pharmaceutical Specialties Certification in at least one discipline (e.g., BCPS, BCACP, BCGP) required within 2 years
MTM Certification preferred.
