The Clinical Pharmacist is responsible for the direct care of patients, primarily in the area of anticoagulation management; initiating, modifying, discontinuing and monitoring warfarin and other anticoagulants according to their scope of practice. Responsibilities include patient education and medication reconciliation, developing and monitoring therapeutic plans, and staff, pharmacy resident, and student training in this area. PERFORMS OTHER DUTIES AS REQUIRED.
1. Graduate of accredited pharmacy school holding current Ohio licensure
2. Doctorate of Pharmacy degree required or 5 years clinical experience
3. Residency training preferred
4. Preferably meets ASHP residency preceptor required criteria
5. Record of scholarship and previous experience in clinical ambulatory setting
6. Board certification preferred
7. Ability to communicate and interact effectively, professionally and courteously with other health care professionals, patients and members of the University community
8. Demonstrates effective and culturally appropriate communications
9. Utilizes effective communication to provide excellent customer services and a positive public image
10. Must be able to withstand pressure due to work volume, interruptions, pace, job variety with an expected degree of accuracy
Physical Requirements:
· Visual acuity of 20/20 or corrected to 20/20 including color vision when applicable;
