Overview

Position Summary: The Nurse Care Manager performs complex discharge planning for patients of different ages and developmental levels with serious illnesses. Focuses on patients with complexities related to finance and/or resource utilization. Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team.

Minimum Qualifications: 3+ years nursing experience with advanced level pediatric clinical experience and utilization management and/or discharge planning experience. Thorough knowledge of managed care, utilization management, discharges planning, and continuous quality improvement concepts. Computer Literacy. Strong interpersonal and negotiation skills. Fluency in both English and Spanish desired.

Education/Licensure/Certifications: Bachelor’s Degree in Nursing required. Master’s degree in Nursing, Health Care Administration, or related field preferred. Current Registered Nurse licensure in the State of

California and AHA CPR/BLS certificate required.

Pay Scale Information

USD $99,424.00 - USD $171,506.00

Final determination of a successful candidate’s starting pay will vary based on a number of factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. Children's Hospital Los Angeles is a nationally ranked, freestanding acute care children's hospital in the East Hollywood district of Los Angeles, on Sunset Boulevard at the corner of Vermont Avenue. The hospital has been academically affiliated with the Keck School of Medicine of the University of Southern California since 1932 and the hospital features 401 pediatric beds. The hospital provides comprehensive pediatric specialties and subspecialties to infants, children, teens, and young adults generally aged 0–21 throughout California and the west coast. The hospital also sometimes treats adults that require pediatric care. The hospital has a rooftop helipad and is an ACS verified level I pediatric trauma center, one of a few in the region.
