Work Schedule/Hours: 7 on/ 7 off - 7pm to 7am

Job responsibilities include those listed in competency document.
  1. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers, in accordance with our Service Excellence Standards and Core Values.
  2. Key Duties/Responsibilities of Provider as a Hospitalist at McLeod Health Cheraw:
    1. Practice evidence-based medicine and follow national quality measures for the delivery of patient care. Provide supportive documentation when clerical judgement or the situation necessitates a deviation from national measures.
    2. Admit unassigned adult (age 16 or over) patients presenting through the Emergency Department.
    3. Accept direct patient admits (age 16 or older) from Community physicians in accordance with the policies and directives of McLeod Health Cheraw.
    4. Pre-op clearance for inpatient surgical cases.
    5. Consultation on medical management with primary care providers and specialists as requested.
    6. Consult services for specialists.
    7. Evaluation of potential admissions upon request.
    8. Effective management of resources such as ancillaries, medications, etc.
    9. Participate in daily multi-disciplinary patient care conferences for utilization review and discharge planning. Communicate discharge plan to patient and family and staff on admission and throughout hospital stay.
    10. Conduct individualized patient and family care conferences for patients with length of stay five (5) days or greater or for complex family and discharge planning discussions.
    11. Length of stay management.
    12. When rendering on-site professional Hospitalist services during a Day Shift, respond to resuscitation codes and be a member of the Rapid Response Team.
    13. Maintain service standards for medical record documentation and completion.
    14. Manage the care of patients in McLeod Health Cheraw Swing Bed Unit. Actively participate in the Swing Bed Patient Care Conference.
    15. Assist in the evaluation and proved recommendations on equipment and supplies needed for inpatient care and treatment.
  3. Responsible for Value-Oriented Care of Patients:
    1. Prioritize and respond in a timely manner to requests for service in such a way as to provide a no-hassle referral and consultative service.
    2. Perform a proper and high value evaluation and follow-up of patient problems requiring current hospitalization and/or follow-up.
    3. Minimize marginal yield testing and therapy in patients through use of clinical skills appropriate for the particular place of the patient in his/her continuum of care.
    4. Follow appropriate hospital developed protocols and internal quality improvement initiatives.
    5. Make appropriate referrals and seek optimal communications concerning the care of the patient. When acting as primary provider of in-patient care, optimize the value of other consultant care and workups to the appropriate place in the continuum of care. This will not affect or influence the clinical decision-making of the physician but is included to explain communication standards.
  4. Role Model Professional Behavior:
    1. Assist other colleagues to achieve the goals of the Ethics Statement of the Medical Staff.
    2. Represent the ideals of the profession and tradition during daily activities with patients, families, staff and the public.
  5. Systems Approach to the Delivery of Care:
    1. Recognize that an individual patient is always somewhere on a continuum of care and that each setting of care has the highest value for a patient with certain characteristics.
    2. Work with individual patients and consults in concert with other Medical Staff members and McLeod Health Cheraw employees to place patients in appropriate value setting along the continuum of care.
    3. Assist McLeod Health Cheraw Departments in system improvements and root cause solutions for improved delivery of patient care.
    4. Communication of patient hand-off to the next phase of the continuum of care is of utmost importance and will be performed by the appropriate timely verbal and written documentation. Hand-off communication includes a physician-to-physician agreement of treatment plan including fulfillment of core measures requirements.
  6. Routine Medical Staff Activities:
    1. Appointed and keeps appointment as Active Member of the Medical Staff.
    2. Subject to Bylaws, Rules, Regulations, and Procedures of the Medical Staff.
  7. Responsible for Value-Oriented Sub-Specialty Consultation Care of Patients:
    1. Prioritize and respond in a timely manner to requests for service in such a way as to provide a no-hassle referral and consultative service.
    2. Perform a proper and high value evaluation and follow-up of patient specialty problems requiring current hospitalization, out-patient practice and/or follow-up.
    3. Minimize marginal yield testing and therapy.

Qualifications/Training:

Licenses/Certifications/Registrations/Education
  • Completion of an accredited Physician Assistant program.
  • Active Physician Assistant License in the state of SC verified by NCCPA.
  • BLS and ACLS Certification.
