Job Summary:

Responsible for providing leadership oversight and direction for the coordination of case management functions across the region's continuum of care. Leadership oversight includes meeting/exceeding expectations established within the Care Management Dashboard, quality improvement activities related to transitions of care, keeping within Riverside Health System services, and regulatory and accreditation compliance. Collects and analyzes data from internal and external sources to ensure successful outcomes of case management functions. Collaborates with key stakeholders to effectively design and implement care management activities, programs and transition of care across the RHS continuum. Responsible for bed control in 4 nursing facilities. Responsible for developing and achieving regional goals with guidance from Facility and Corporate leadership, as well as promoting professional standards in accordance with health system policy, case management professional societies, local, state and federal regulations. Collaborates with community resources, both internal and external agencies, continuum of care providers and physicians to optimize care from clinical and administrative perspective. Actively participates in the Middle Peninsula Regional Resource Council. Supervises staff including staff evaluations, counseling and mentoring. Leads regional care management collaboration efforts. Maintains knowledge of Utilization Management, oversees UM in the region and manages results.

Education:

BSN, BHA, BBA or related degree required.

Certification/Licensure Requirements:

Valid, current RN Licensure in good standing with VA State Board of Nursing.

Experience:

Minimum 5 years working experience in Case Management required; experience must also include 5 years working experience in management and/or supervisory that includes managing teams in a health care, hospital, physician practice or comparable environment. Experience building training and/ or auditing programs for front line staff preferred.
