Education: Masters or doctoral degree mental health counseling program
Experience: 3 Years of Clinical experience
Additional Details: Knowledge of clinical and social factors that affect a patient’s chronic disease management. Excellent analytical skill.
License: Florida state license. Valid Florida Driver’s License Required
Other Requirements: Exceptional organization and interpersonal skills. Demonstrates professional and effective communication skills, including written, verbal and nonverbal. Ability to work well with people of various ages, backgrounds and ethnicities. Proficiency in communication technologies (email, cell phone, etc.) of coordinating multifaceted workload. Must have excellent written and verbal communication skills. Must have in depth knowledge of clinical therapy modalities. Must demonstrate knowledge of the normal principles of growth and development, be able to assess and interpret both cultural and age/disability specific intervention into effective treatment planning in areas of chronic mental illness, geriatric, children and adolescent, and chemical dependency populations. Must be flexible and able to perform in stressful situations.
Works in collaboration and partnership with chronically ill or “high-risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach to provide linkages for the various health and social needs of the patient. Provides assessment, treatment, group and individual therapy, and support services. Develop in conjunction with clinical team, an individualized treatment plan to address needs of the patient/significant others to support recovery. Work with individuals and groups to promote optimum mental and emotional health associated with chronic disease, depression, addictions and substance abuse, stress management; self-esteem; and aging.
