Behavioral Health Services (BHS) provides a full range of services for persons with severe and persistent mental illness and/or individuals with emotional well-being needs to achieve a maximum level of independence, community integration, and satisfaction with life. The Integrated Care Nurse-RN is a key associate at Hope Network whose primary responsibilities include: the maximum quality, efficiency, and economy of all nursing operations of Hope Network Integrated Care. This includes the facilitation and/or coordination of the medication administration. Development of individualized health treatment plans and corresponding monitoring and/or intervention guidelines for staff. The RN acts as the liaison between the team and external entities as well as internally with the psychiatric provider. The RN also provides education as requested to improve health literacy of the IC team.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

1. Regular and predictable attendance is an essential requirement of this position.
a. Ensure other licensed individuals whom are functioning under RN license are competent in tasks delegated per their scope of practice. RN may delegate tasks to other licensed medical individuals (LPNs and MAs) as needed and appropriate.
b. Maintain knowledge base of current nursing practices and procedures as they become available.
c. Coordinates and implements infection control procedures and other quality assurance procedures to protect individuals, staff, and their associates. Track program infections on the Hope Network infection tracking system.
d. Ensure program’s medical related compliance with state and federal regulations (i.e. MIOSHA, OSHA, etc.).
e. Ensures all necessary consents, releases, and regulatory standards are met and completed to insure continuity of care, delegates to the Care Coordinator as necessary.
a. Be familiar with relative corporate and departmental policies and procedures, ensuring that these policies and procedures are followed.
b. Develop and provide necessary health/medical trainings to direct care staff as directed by accreditation and regulatory standards. This includes participating in teaching the Meds & Vitals portion of the annual refresher when needed.
c. Provide training to residential staff in all health care related areas as needed. Develop training programs and obtain training aids. Training could include verbal, visual by tapes, slides, film and written material.
d. Coordinate and monitor the administration (including periodic medication passing checks) procurement, and evaluation of individual medications.
e. Provide leadership, support, and direction to staff to facilitate communications, problem solving, and program development.
f. Attend the monthly regional nurse meeting.
g. Be a part of the nursing on call rotation. Follow the nursing on-call standards for documenting concerns addressed.
h. Assist with the interviewing, hiring and training of applicants/new hires for positions as requested
i. Participate in appropriate program meetings designed to develop program goals and monitor quality assurance for optimal professional nursing services to program areas.
j. Participate in the needs of agency or community committees, task forces, and other related meetings and activities as assigned by supervisor.
k. Act as a source of reference regarding the electronic medical record. Be available to answer questions and train staff as needed
l. Assist the nursing students and their instructor during their nursing program psychiatric rotation
4. Care Coordination
a. Perform at least one hospital visit for each consumer inpatient episode. Review medical information from the hospital and advocate for the best patient care and outcome.
b. Resposible for the coordination of psychiatric provider services.
c. Responsible for obtaining all records necessary for current appointments to include lab results, referral records, current medications and delegates to the Care Coordinator as necessary.
d. Responsible for ordering medications as delegated by the psychiatric provider through electronic and verbal methods
e. Responsible for the coordination external medical services as needed
f. Communicate with all affiliated physicians relative medical issues.
a. Monitor and treat chronic and emergency medical conditions and illnesses within the advisement of affiliate physicians. Develop evidence-based health treatment plans relating these conditions and provide staff with information to carry out these plans.
b. Act as general consultant on medical and health related issues of the individuals.
c. Assess individuals health needs by performing admission and annual nursing assessments and develop evidence-based total health treatment plans using nursing process for assigned residents per regulatory standards and physician instruction. Monitor and schedule laboratory tests and examination in conjunction with program policy.
d. Documents medical treatment and health status in individual records for continuity of care and treatment team reference per regulatory standards.
e. Monitors and tracks consumer response to medication changes. If monitoring guidelines need to be followed, the RN creates the guidelines for staff to implement.
f. Report all medical concerns and treatments needed to the appropriate Case Manager, Program Manager, Program Director and/or other appropriate team members.
g. Provide input regarding consumers for medication review with psychiatrist as needed.
h. Document medical treatment and health status in resident records for continuity of care and treatment team reference.
i. Reviews all external provider records and ensures ecare diagnoses, non-hope meds, allergies and adverse reactions are updated as needed
Educational / Talent Requirements:
1. Degree or diploma of nursing
2. Licensed Registered Nurse in the State of Michigan.
3. Valid Michigan driver’s license required according to Hope Network policy.
4. Positive attitude and communication skills.
Work Experience Requirements:
1. Experience in providing medical services in a mental health setting
2. Experience in providing medical services in a long-term care setting
3. At least 5years experience as a nurse, 10 or more years preferred
