Provides pharmaceutical services, including but not limited to verification, dispensing, monitoring, and education, to all patients. Reviews and maintains inpatient and outpatient medication profiles to improve medication safety and medication adherence. Facilitates prescription delivery through the Meds to Beds program. Works independently, communicates effectively, and is able to prioritize responsibilities. Interacts with other health care providers to reduce barriers to medication access and improve patient outcomes.


  • Collecting medication histories, assisting providers with medication reconciliation, verifying and dispensing outpatient prescriptions, performing medication counseling, and resolving barriers to medication adherence.
  • Performs all functions relating to outpatient pharmacy medication verification and dispensing, controlled substance inventory management, patient counseling, and delivery of immunizations.
  • Shows understanding of and complies with 340B and Medicare/Medicaid standards during the billing process.
  • Oversees and supervises support pharmacy personnel.
  • The Outpatient Infusion Pharmacist will work as part of an interdisciplinary team to ensure continuity of care.


  • Doctorate of Pharmacy degreerequired
  • Licensed and Registered in New York State
  • CPR/Basic Life Support (BLS) certified
  • Active NYS Pharmacist Immunization certification preferred
  • In compliance with New York State CE Credit requirements
  • PGY-1 residency trained preferred
  • 2 or more years of pharmacy experience as a pharmacy intern or pharmacistpreferred
  • Equipment - Basic knowledge of computer applications (i.e. MS Word, E-mail, etc.)
  • Demonstrates strong communication skills, interpersonal skills, compassion, and empathy.
  • Knowledge for the field of work, including departmental, institutional, state, and federal policies and regulations.
  • Working weekends and holidays isrequired
