About the Opportunity
INTEGRIS Health in Oklahoma City is looking for a board certified or board eligible Hospice and Palliative Medicine (HPM) physician to join our growing interdisciplinary program.
We are expanding our supportive care services to include a robust system palliative medicine program to care for patients of all ages with serious illness and chronic disease. Our goals are to improve quality of life and overall health care experience by reducing symptom burden and psychosocial distress while continuing curative therapies and assisting the primary physician and other specialists to more completely care for the whole person and family along with the disease process.
Reporting to the INTEGRIS Hospice and Palliative Care Program Director, this physician position will be responsible for provision of palliative care consultations, via in person and telemedicine visits, within INTEGRIS hospitals and clinics as well as supervisory responsibilities for the APRNs dedicated to the palliative care team.
Duties will include consulting with attending physician and the interdisciplinary team to establish a written plan of care and facilitate access to appropriate supportive care services. In addition, the physician will participate weekly in the interdisciplinary team meetings to review active cases and documentation.
As an arm of Integris Supportive Care Services, Integris Palliative Care works side by side with Integris Hospice when transition to end of life care is needed. Integris Hospice has been well established in the OKC community for over 30 years and has operated a free-standing 12 bed inpatient Hospice House for the past 10 years. The HPM physician will rotate call coverage with the palliative care team and hospice team.

The Physician provides direct patient care. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires.
The Physician responsibilities include, but are not limited to, the following:
* Thoroughly evaluates and manages each patient appropriately and documents information completely by use of written notes or dictation on a timely basis
* Identifies relevant medical issues accurately and uses appropriate procedures and testing
* Completes and maintains problem list, medication list, disease management forms, preventative maintenance flow sheets and other medical record forms
* Participates in the quality improvement and medical management plans as directed by the Medical Operating Committee
* Provides direction to staff members for an efficient and smooth running team
* Ensures fiscal viability of the practice
* Complies with policies and procedures if applicable.
The Physician is responsible for patients.

Moderate exposure to hazardous risks, including potential for exposure to infections and communicable diseases, blood and body fluids, electrical equipment, chemicals.
