Job Description

Responsibilities:

Oversee and manage the care provided to patients in various settings, including acute care, home health, and home care.
Ensure that all care provided meets the highest standards of quality and is in compliance with relevant regulations and guidelines.
Develop and implement care plans for patients based on their individual needs and goals.
Conduct regular documentation reviews to ensure accuracy and completeness of medical records.
Collaborate with other healthcare professionals, such as physicians, nurses, and case managers, to coordinate patient care and ensure seamless transitions between different levels of care.
Stay up-to-date with advancements in medical knowledge and technology to provide the best possible care to patients.

Skills:

In-depth knowledge of anatomy and physiology to understand the complexities of the human body and its systems.
Strong managerial skills to effectively lead a team of healthcare professionals and ensure efficient operations.
Experience in acute care settings to handle complex medical cases and emergencies.
Familiarity with home health and home care services to provide appropriate care in a patient's home environment.
Proficient in documentation review to ensure accuracy and compliance with regulatory requirements.
Excellent communication skills to effectively collaborate with interdisciplinary teams and communicate with patients and their families.
Strong case management skills to coordinate care across different healthcare settings.

Requirements:

Bachelor's degree in a related field (e.g., nursing, healthcare administration) required; Master's degree preferred.
Valid licensure/certification as required by state regulations (e.g., Registered Nurse, Licensed Practical Nurse).
Strong leadership abilities with a focus on providing compassionate and patient-centered care.
