HCA is searching for anArea Director of PAC (Post-Acute Care) Network Partnerships to join our team. The Director of PAC Network Partnerships develops and maintains an ongoing collaborative relationship with PAC providers based on mutual respect, high quality outcomes and patient satisfaction. The Director of PAC Network Partnerships works to continually shape the Networklandscape by offering objective data and education to all stakeholders in the market. The Director of PAC Network Partnerships plays a vital role in achieving positive patient outcomes in the market including but not limited to: improving Skilled Nursing Facility (SNF) length of stay (LOS), increasing the efficiency of PAC providers delivery of care, increasing discharges to Preferred PAC Providers, monitoring of readmission trends and tracking quality of care measures across all Preferred PAC Providers.Duties Include but are not limited to:

  • Responsible for establishing and maintaining relationships with PAC providers and supporting the patient centric model.
  • Demonstrates expertise in coordinating, directing and managing all aspects of PAC provider networks within the region.
  • Executes Corporate/Group/Division/Facility and Bundles goals by leading initiatives, meeting deadlines, coordinating resources and providing accountability to the hospitals as well as the Preferred PAC Providers.
  • Responsible (with local hospital management) for the hiring, training and supervision of each facility PAC Care Manager.
  • Provides regular onsite reviews of PAC provider operations to ensure quality patient care and staff support and communicates status to leadership.
  • Serves as official Post-Acute Care Network consultant to Division leadership.
  • Works with Corporate Bundles team and coordinates with HCA resources to ensure efficient and effective operations for the Division's post-acute networks.
  • Ensure that relevant data is gathered and monitored regarding the PAC provider community within the market.
  • Monitors database of PAC provider metrics (e.g. readmission rates, ALOS, publicly reported quality metrics, patient experience, etc.).
  • Coordinates staff, facility and process development activities (scheduling dates/locations, speakers/attendees, contact points, deck preparation, status updates, etc.).
  • Creates hospital-approved provider facing educational and summary documents.
  • Ensures all necessary logistics obtained to support a successful Preferred PAC network (contacts, EMR access, onsite access, etc.).
  • Responsible to maintain positive working relationships with PAC providers and hospital staff to build a high quality, highly collaborative PAC network base.
  • Serve as main point of contact for PAC providers.
  • Delivers PAC provider and hospital performance information at a regular cadence
  • Holds PAC providers accountable to:
    • Professional collaboration (based on collaboration agreement)
    • Timely delivery of clinical information
    • Supporting patient and family access to the Transitional Care Coordinator
  • Evaluates PAC provider receptiveness and quality
  • Responsible to lead PAC network issue resolution
  • Identifies challenging situations early and ensures appropriate action taken to mitigate escalation.
  • Support PAC providers as they have questions / concern / suggestions
  • Solicits feedback as appropriate (focus groups, survey, coalitions)
  • Works to resolve PAC provider complaints at a local, regional and national level
  • Develop ongoing support materials for PAC providers
  • Provides ongoing education to PAC providers that promotes quality service and collaboration
  • Monitors and works to improve:
    • Hospital / PAC Provider communication
    • SNF Length of Stay
    • Avoidable Readmission
    • Downstream in-network PAC utilization
  • Reports challenges with care coordinators in the field
  • Works closely with Corporate Bundled Payment leadership
  • Analyzes data for trends in PAC performance on a market level
  • Supports new and existing strategic PAC network initiatives and programs
  • Communicates PAC network initiatives to PAC providers
  • Supports company specific initiatives as appropriate
  • Supports the onboarding of new PAC network colleagues through mentorship
  • Works with internal resources on development of Preferred PAC Provider Networks
  • Supports and implements pay for quality programs, as applicable
  • Executing direct contracts with post-acute care providers where applicable
  • Document all provider encounters

Qualifications:

  • Degree in therapy, nursing, social work, or healthcare related field.
  • 3+ Years in a customer facing or Account Management Role preferred
  • Experience in Acute and Post-Acute Care Continuum required
  • Executive communication and presentation skills required
  • Data analytics and reporting experiencepreferred
  • Proficiency with Microsoft Office applications
  • Effective negotiation and influencing skills
  • Detail oriented
  • Ability to build and foster relationships inside and outside the organization
  • Ability to independently plan, organize and prioritize work
  • Willingness to work toward team objectives
  • Ability to manage remote, direct reports and/or team members
  • Experienced in managed care, customer service, and provider relations
  • Strong brand promotion and management skills
  • Valid Driver’s License
