Job Description

QUALIFICATIONS & EXPERIENCE

1. A bachelor's degree in science in Nursing required. Master's Degree in nursing or healthcare administration or related field preferred.

2. Registered Nurse in the state of Michigan.

3. Experience working in or leading clinical teams and operations with strong clinical acumen and experience delivering patient care.

4. Three (3) to five (5) years of progressive leadership experience in a related healthcare organization with preferred experience in clinical operations.

5. Experience with palliative, end-of-life, home, or community-based health care preferred.

6. Experience working with multi-disciplinary teams, building consensus, and leading people and projects forward.

7. Present a negative TB skin test and/or chest X-ray and other tests as required by Centrica Care Navigators.

8. Maintain a valid Michigan driver's license and has the availability of a reliable, licensed, and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

9. Remains up-to-date on all routine immunizations required by LARA and agency policy.

SUMMARY OF JOB RESPONSIBILITIES

Responsible for leading and managing the coordination, planning and day-to-day leadership of patient care and operations provided by the nurses, home health aides, social workers, chaplains for Centrica Hospice and Hospice of Michigan SW in compliance with all regulatory standards and agency policies. Develops, plans, implements, and evaluates nursing, home health aides, social workers, and chaplains to ensure the quality and safe delivery of care, treatment, and services.

RESPONSIBILITIES AND DUTIES

Primary Duties

  1. Develops, implements, and evaluates goals and standards for the delivery of home care services, which exceed standards.


  1. Provides overall leadership of -patient care and operational service provided by nursing, home health aides, social workers and chaplains.


  1. Knowledgeable of Hospice Medicare Benefit and ensures compliance with Medicare Conditions of Participation, State Licensure Rules, and all Federal and State regulations.


  1. Delegates the maintenance of appropriate inventory of medical supplies and equipment across all home care sites for the provision of patient care services. Ensures cost-effective ordering of supplies that takes into account expiration dates and ease of use by clinical staff.


  1. Ensures that quality and delivery of nursing and applicable other services are available 24 hours a day/ 7 days a week. Ensure continuity of care throughout service delivery.


  1. Evaluates home care clinical services through a variety of means such as home visits and record reviews. Responsible for the quality of services and performance improvement initiatives impacting the home care clinical area. Responsible for the sustainment of the clinical quality initiatives.


  1. Participates in public relations and community activities that promote the Hospice agency's role as an effective member of the Hospice and health care delivery continuum.


  1. Develops ongoing positive and professional relationships with contracted partners and other related bodies, including referral sources.


  1. Shares after hours, weekend, and holiday administrator on-call responsibilities.


  1. Responsible for the mitigation, preparedness, response, and recovery activities as outlined in the organization's Emergency Management Plan.


  1. Monitors and ensures adherence to Centrica Care Navigator's and Hospice of Michigan's standards for provision of care and ensures that documentation complies with local, state, and federal regulations.


  1. Provides strong, visible leadership and direction through compelling communication of the agency vision, values, strategic initiatives, and sets clear staff expectations.


  1. Creates an environment of accountability by constructively and appropriately challenging employees to deliver agreed upon outcomes/results and models desired values and behaviors.


  1. Develops, recommends, and administers the home care clinical operations policies and procedures.


  1. Identifies, gathers, interprets, and monitors data and other operational and quality indicators as necessary for decision making purposes, including changes that affect staffing, revenue, and expenses.


  1. Assists leadership with implementation, monitoring, and evaluation of clinical contractual agreements.


  1. Oversight of agency-wide Infection Control plan in coordination with the direct reports.


  1. Oversight of the electronic medical record and associated workflow activities critical to the home care clinical arena.


  1. Works closely with the Medical Director and clinical leadership to ensure appropriate admissions, discharges, provision of care, and timely certifications.


  1. Designates leadership of Interdisciplinary Group Meetings to develop and facilitate coordination of care.


  1. Promotes a safe environment of care for agency personnel, volunteers, visitors, and the patient/family/caregiver through adherence of best safety practices.


  1. Coaches, mentors, and develops employees and holds them accountable to meet agency/supervisor expectations ensuring that both effective performance and delivery of quality care are reinforced.


  1. Responsible for the quality of services impacting the home care clinical area. Responsible for the sustainment of those initiatives.


Leadership Duties

  • Assists staff to develop skill in planning work assignments.


  • Attends meetings of the Board of Directors and Standing Committees as requested.


  • Provides information, statistics, reports, and records for purposes of evaluation and reporting on departmental activities.


  • Responsible for creating a work environment that assists staff in completion of tasks in meeting strategic goals.


  • Monitors and ensures compliance with applicable local, federal, and state regulations, OSHA, infection control, and compliance guidelines, privacy protections, and protected health information.


Leadership & Supervisory Duties

  • Works to understand, interpret, adhere to and apply Agency policies and procedures accurately and constructively.


  • Provides oversight of responsible areas for the Agency's budget.


  • Hires new staff in accordance with established procedures. Orients or delegates orientation of new employees and ensures that employees have adequate ongoing training and development.


  • Establishes and communicates job responsibilities, performance standards, priorities and expectations to staff.


  • Routinely observes and documents both good and poor job performance and conducts timely performance evaluations according to set standards.


  • Informs and works in consultation with Human Resources on employee discipline issues (i.e. policy violations, performance issues) and terminations.


  • Promotes and ensures adherence to Centrica Care Navigator's Workplace Safety Program.


  • Ensures that staff are properly licensed and/or credentialed.


  • Provides oversight of staff hours, competencies, education, and evaluation of the department/area.


Duties

  • Actively participates in agency-wide performance improvement activities.


  • Adheres to Centrica Care Navigator's standards of personal and professional conduct.


  • Demonstrates an understanding of and commitment to the agency philosophy of care, mission, and vision.


  • Ensures that all necessary information is shared with appropriate personnel.


  • Adheres to Centrica Care Navigator's Workplace Safety Program.


  • Understands and complies with Hospice Medicare and Medicaid conditions of participation.


  • Complies with applicable local, federal and state regulations, Quality Partners standards, compliance guidelines, privacy protections and protected health information.
