DIRECTOR OF CASE MANAGEMENT DES PERES HOSPITAL
: The Director of Case Management is responsible for developing, implementing, and evaluating the strategic plan for the organization-wide Case Management process. Provides operational supervision of the Case Managers and Social Workers. Coordinates and supports ongoing problem solving. Establishes tools, processes, and systems to optimize the Case Manager and Social Worker’s roles.
*1. Establish, implement and evaluate a strategic plan for the department that takes into account internal and external factors.
*2. Create data priorities and report formats that display critical outcome factors and reviews the finding with the executive team and provide feedback to the department staff regarding the Care Coordination sensitive data findings.
*3. Identify appropriate staffing levels, assignments, and deployment for the department and establish role descriptions and performance criteria for staff members.
*4. Develop and administer the budget for the department.
*5. Identify organizational needs and concerns in payer-provider contracts and contributes to related negotiations.
*6. Develop and maintain effective relationships with hospital department heads, administrative staff, and physicians. Develop and/or contributes to an effective outcomes management process. Establish effective relationships with external agencies providing services to the organization, i.e. vendors.
*7. Direct utilization review activities for the hospital, interfacing with external payers and review companies to determine appropriate medical necessity of admissions and continued hospitalization.
*8. Resolve conflicts related to discharge planning with patients/families, external agencies and payers.
*9. Establish policies, procedures and protocols for departmental functions.
10. Performs other duties as assigned.
EDUCATION AND TRAINING:
Graduate of school of nursing. BSN preferred.
Eligible for or currently licensed as a Registered Nurse in the State of Missouri
Minimum five years experience in case management.
Prior experience in a management or administrative position preferred.
Familiar with discharge planning, utilization review, or related experience preferred.
SKILLS AND ABILITIES:
Evaluative skills necessary for patient assessment.
Decision making skills required for planning/implementation.
Clinical reasoning and judgment skills necessary for evaluation of nursing interventions and for safety of patients and self.
Excellent oral and written communication skills to interact with patients, visitors and staff.
Ability to manage time, organize daily schedule to meet productivity standards, and cope with professional stress.
