Position Summary:
The  Director of Clinical Integration works collaboratively to drive the design, implementation and adoption of system wide strategies and workflows that support high priority system objectives, population health management and value based purchasing contract requirements.

The Director oversees multiple programs – including a team of program coordinators who manage Clinically Aligned Resource Effectiveness (CARE) groups, and teams of Supervisors/Managers, Registered Nurses, and care coordinators who support population health efforts, Medicare risk adjustment, a standardized Annual Wellness Visit platform, and value based care measures in the primary care setting.


In partnership with physician, finance, and nursing leads, the Director is responsible for the successful planning and implementation of the CARE group program and associated performance improvement deliverables. The Director also partners with the Physician Executive & VP – Clinical Integration to support efforts to align and integrate clinical campaigns across carelines and the system, ensuring strategic operational alignment and implementation of these system-wide initiatives; financial viability and sustainability; performance improvement deliverables are best in class; and quality performance is prioritized and matched with value based contractual requirements/resources.

Work will involve the reduction of variation in highly complex clinical care pathways, developed at the system level, and will involve collaboration with senior-level sponsors and stakeholders to ensure appropriate development and transition into deployment and adoption.

Minimum Requirements:
Eight (8) years of experience in project management and clinical operations/practice management in progressively more responsible positions
Five (5) years of experience in a supervisory or lead capacity
Strategic, business and program development experience
Experience in value based reimbursement & population health strategies

Education/Licenses/Certifications:
aster’s degree in business or healthcare related field
Project Management Professional (PMP) Certification from the Project Management Institute (PMI) preferred
