Planned Parenthood of Greater New York is seeking a Director, Care Management to join our Project Street Beat team. The Director, Care Management position will be primarily based at our PSB office located at 349 E 149th St., Bronx, NY 10451 and will assume responsibility of NYC and Nassau County.
This position is responsible for clinical and programmatic oversight of Project Street Beat’s (PSB) integrated case management and supportive services including consumer engagement and retention, case management, wellness support, and Health Home Care Management services for PSB’s centered populations, including people who are at increased risk for exposure to and those living with HIV and other chronic conditions, unstably housed and homeless, abuse substances, and engage in transactional/ survival sex. Working in collaboration with PSB’s Vice President and Associate Vice President, the Director of Care Management provides senior-level leadership for Project Street Beat. Assumes overall responsibility for program leadership in AVP’s absence.

Proof of immunization or immunity to certain communicable diseases (including influenza during the flu season) and testing for tuberculosis. These certifications are required by the NYC DOHMH Health Code, NYSDOH and OSHA.
