ConcertoPACE of Los Angeles is looking for a mission-driven, patient experience-oriented, servant leader to manage ConcertoPACE clinical operations serving the healthcare needs of the frail elderly. Reporting to the Executive Director with a dotted line to the Medical Director, the role is accountable for successful operations of the on-site primary care clinic and for enabling in-home care including in-home medical care and home nursing. The role is responsible for ensuring high standards for access to care, quality of care (in partnership with other leaders), patient experience, and the implementation of new processes and programs within the clinical operations context.

Joining the leadership team at an early stage, the Clinical Services Manager will shape the operations and culture of the clinic and home nursing team. The Manager will be responsible for building a patient-centered team comprised of home care nurses, registered nurses, licensed vocational nurses, certified nursing assistants, medical assistants, and other staff. As the PACE program grows, the Clinical Services Manager is accountable for scaling and ensuring the success of his or her team.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • The principal duties of this position shall consist of, but are not limited to, the following:
  • Accountable for the successful operations of the PACE Clinic and Home Care teams. Ensures enrolled participants have adequate access to high-quality, patient-centered care. Orients team to provide a high degree of customer service, striving to delight each participant with their experience of ConcertoPACE.
  • Supervise the Clinic and Home Care team members including but not limited to CNAs, PCAs, and MAs. Responsible for empowering the team to be successful through hiring, training, team building, and performance management of all team members. Monitor team performance against goals and standard operating procedures, including appropriate use of the EMR.
  • Collaborate with providers to ensure providers are empowered to deliver high-quality care to each participant during each visit.
  • Support execution of operational processes as needed. This includes but is not limited to front office operations, clinic administrative processes and coordinating delivery of home care with employed staff and agencies.
  • Participate in Quality Program and makes recommendations based on expertise to provide the highest standard of care.
  • Implement new processes and programs within the clinical operations context. Develop and train team members on standard work to ensure positive patient impact and team success. Collaborates with other leaders and disciplines to develop appropriate policies and procedures.
  • Coordinate activities necessary to provide a high-quality environment of care by managing facility medical equipment, clinic licensure, and following infection prevention procedures. This includes inspection, repairs, and maintenance of equipment, implementing and monitoring safety logs, maintenance/renewal of appropriate clinic and lab licensure (i.e. CLIA), and training staff on appropriate infection prevention protocols.
  • Maintains fiscal accountability for the clinic and home care teams by managing to budget in the areas of payroll/overtime, team expenditures, and consumable medical supplies.
  • Coordinates with contracted vendors and providers to successfully serve participants.
  • Ensure the PACE clinic and Home Care operations comply with all federal and state regulations and ConcertoPACE corporate policies, procedures, and guidelines. Participates in the investigation of Level II reports and root cause analysis to state and CMS officials.
  • Actively develop, participate, and monitor emergency preparedness and business continuity plans and drills to ensure a state of readiness and safe environment for all participants, staff, and visitors.
  • Other duties as assigned.


QUALIFICATIONS

  • Bachelor’s Degree preferred.
  • Management and leadership skills required. Three years of prior experience as a staff supervisor/ clinic manager serving the geriatric population preferred.
  • Inspires a patient-centered culture among direct reports and team members. Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff, and the public.
  • Experience developing and implementing policies, procedures and other written processes.
  • Ability to actively participate in strategic planning for both the Home Care Department and the overall PACE program.
  • Ability to communicate clearly and effectively, both verbally and in written formats.
  • Proficient in Excel and Microsoft Office programs, including Outlook.
  • Ability to work independently with minimum supervision.
  • Skilled in identifying and recommending problem resolution.


Base Salary/ Wage Range $85,000 to $95,000 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
