    The Clinical Pharmacist is responsible for providing pharmacy services from the Central pharmacy, outpatient pharmacy, and Unit-Based Pharmacy program. The Clinical Pharmacist is responsible for reviewing, clarifying and making recommendations for all medication orders. The Clinical Pharmacist is responsible for coordinating and checking all of the activities of the Certified Pharmacy Technicians. The Clinical Pharmacist will provide drug information and education to hospital personnel and the community as well as assisting with quality improvement initiatives. Ensures compliance with State and Federal regulations concerning Controlled Substances. Also provides age and developmental appropriate care toinfants, children, adolescents, and adults.

    Primary Functions:

    1. Previews and interprets medications orders for accuracy using sound principles, accounting for pharmacodynamics and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients within areas of direct responsibility and assisting others in the department with the same.

    2. Carries out rules and regulations pertaining to the professional policies of the pharmacy department.

    3. Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians and practioners on issues concerning drug therapy, the inherent toxicity of drugs, drug-

    4. Responsible to provide accurate, adequate, and timely drug information to the professional staff and to patients and their families.

    5. Coordinates the activities of assigned certified pharmacy technicians engaged in the preparation, labeling and delivery of medications in accordance with federal, state, and local laws and regulations pertaining to the dispensing of drugs, controlled drugs, investigational drugs, and the maintenance of required records.

    6. Facilitate orientations and educational programs for the medical staff and residents on the appropriate utilization of Pharmacy Services.

    7. Participates in the quality improvement and medication use review activities of the department. Collects data; conducts quality monitors and inspections; maintain logs, records, and other documentation as assigned.

    8. Reads, extract, and interprets information in patient’s charts accurately.

    9. Promote the use of the Nemours/AIDHC drug formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary dugs when necessary.

    10. Mentor pharmacists-in-training including but not limited to Pharmacy

    Performance Skills:

    1. Monitor drug therapy to evaluate appropriateness of use, dose, dosage, form, regimen, route, therapeutic duplication, and drug / food interactions.

    2. Ensuring that the practice of pharmacy is conducted in accordance with accepted ethical and professional standards.

    3. Effectively communicates in written formats both verbally and non-verbally, resolving drug distribution related issues..

    4. Document clinical interventions and cost avoidance as part of the departmental documentation program.

    5. Nemours/AIDHC Team Player

    6. Continuously assessing and improving the pharmacy’s performance.

    7. Self motivates, stays on task, and works independently, to complete all required CBL (Nemours University) by

    November 30, of each calendar year.

    Work hours: Rotating days, evenings, and holidays and every 4th weekend.

    Education and Training:

    B.S. Pharmacy or Pharm D with Hospital experience. Currently licensed by the State of Delaware or legally eligible.

    Unit dose training, IV, TPN, Oncology and Dialysis experience. Drug Storage, Distribution, and Automation Technology Systems. Pharmacy Software.
