Job Description

Summary:

Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Manager or Supervisor is responsible for the interpretation, evaluation, preparation and dispensing of medications pursuant to a legitimate order in keeping with established policies, procedures, and standards of practice and scope of pharmacy services of the respective affiliate. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and closely reviews the work of pharmacy technicians and ancillary support personnel.

Responsibilities:

Receives patient medication orders as written by a physician or practitioner. Reviews and interprets each order to determine completeness, therapeutic appropriateness as it relates to the patient's clinical status, concomitant drug therapy and diet, and with consideration of the aging processes and human developmental stages (neonate, child, adolescent, adult, geriatric). Contacts the prescriber, or covering prescriber, as necessary to initiate appropriate interventions

to ensure therapeutic efficacy, minimize potential risks of drug therapy, and optimize cost effectiveness.

Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served.

Provide drug information and consultation to prescribers, nurses, and other health care professionals regarding appropriate drug therapy, including drug interactions, side effects, incompatibilities, allergies, medication order discrepancies, drug stability problems, and drug administration. Proactively reviews and approves medication orders in LifeChart including drug, dose, dose form, frequency, and duration of therapy. Ensures relevant patient specific parameters are present in LifeChart and considered (i.e., diagnosis, known allergies, weight, age, etc.).

Supervises work performed by pharmacy interns and technicians to ensure performance standards are maintained and work is in accordance with federal, state, and organizational rules and regulations.

Identifies, documents, resolves, monitors, and prevents drug therapy related problems, including contraindications, duplications of therapy, and drug interactions.

Engages with other clinical pharmacists or clinical pharmacist specialists for complex clinical situations

Integrates patient-focused medication therapy management and pharmaceutical care in accordance with established clinical pathways or guidelines.

Participates in prospective and retrospective pharmacokinetic dosing and monitoring of selected medications and recommends dose adjustments and laboratory monitoring as required.

Ensures compliance with target drug programs, formulary management initiatives, medication management initiatives, and adverse drug event monitoring programs.

Documents and reports all clinical interventions, medication errors, near misses, and adverse drug events in accordance with established protocols.

Participates in emergency code blue response.

Attends and participates in daily medical, multi-disciplinary, and specialty rounds as time allows.

Reviews and approves formulations for sterile products requiring compounding or manipulation prior to dispensing. In accordance with established procedures, completes in

process and final checks for medications prepared by pharmacy technicians. Ensures accuracy, sterility, and stability of compounded products.

Participates in education and training of healthcare practitioners, including pharmacy residents, students, interns, and technicians, physicians, practitioners, nurses, and other care professionals on matters relevant to medication use.

Serves as preceptor to pharmacy residents and students.

Participates in continuous quality improvement activities and total quality management initiatives, including, but not limited to adverse drug reaction, pharmacist clinical intervention, and medication incident reporting programs.

Participates in coordination of transitions of care, as appropriate.

Maintains professional competence through participation in Pharmacy staff development and skills enhancement programs, including but not limited to ongoing independent study, and membership and participation in professional organizations.

BASIC KNOWLEDGE:

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.

Prior to start date, possesses current unrestricted licensure as a licensed pharmacist or as a licensed 90-day temporary pharmacist in the State of Rhode Island. Obtains full licensure prior to expiration of 90-day temporary pharmacist license, then possesses and maintains current unrestricted licensure as licensed pharmacist in the State of Rhode Island.

Must possess and maintain current American Heart Association Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Certifications within 120 days of hire to fulfill emergency code blue response responsibilities as assigned.

EXPERIENCE:

Successful completion of Post Graduate Year 1 (PGY-1) Residency from an accredited program and/or national Board Certification in Pharmacotherapy preferred. Post Graduate Year 2 (PGY-2) Residency from an accredited program in area of specialty strongly preferred, if

applicable. Clinical experience, knowledge, and development may be considered in lieu of formal training.

Effective interpersonal and communication skills, both written and verbal, to interact appropriately with patients, families, colleagues, and others. Effective organizational and presentation skills.

Knowledge of area/specialty specific technology, including, but not limited to, pharmacy information systems, automated dispensing technology, software, equipment, and automation. Experience with MS Word, Excel, Outlook, and web browsers.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Extended periods of time spent standing and walking.

Requires the visual and manual dexterity to operate a computer.

Occasional travel between facilities or sites as required.

INDEPENDENT ACTION:

Functions independently within broad scope of departmental policies and practices.

Refers specific complex problems to supervisor when clarification of departmental policies and procedures may be required.
