Responsibilities: The clinical pharmacist will provide direct patient care as part of a pharmacist-run anticoagulation clinic and diabetes management within the primary care setting. Parkview cares for patients in the Northeast Indiana, Northwest Ohio and surrounding areas. Parkview employs over 650 providers as part of Parkview Physician Group in Primary Care and extensive Specialty Providers. The pharmacists manage anticoagulation via point of care office visits, enoxaparin bridging, patient self-testing, and evaluate and counsel patients on direct oral anticoagulants. The Anticoagulation Clinic is a required longitudinal ambulatory rotation for the PGY1 program. Diabetes management is within a primary care clinic via a collaborative practice agreement. This position may have the opportunity to expand into other ambulatory pharmacy service areas such as population health. Parkview pharmacy has seen tremendous growth in ambulatory with pharmacists in primary care, cardiology, movement disorders, falls, cardiology, population health, and more. Developing and leading quality initiatives that will make a difference across a variety of ambulatory settings is an example of the professional reward that this clinical pharmacist can expect. This position will have the support of other clinical pharmacists, an ambulatory clinical pharmacy supervisor, and a manager of ambulatory services to mentor and facilitate opportunities. Parkview Health has provided specialty pharmacy services in a variety of areas for over 20 years. Parkview offers a progressive pharmaceutical care environment in a variety of practice settings, including inpatient pharmacy, interdisciplinary rounding, ambulatory specialty clinics, a surgical/critical care satellite pharmacy, and a cancer satellite pharmacy. Parkview Pharmacy leadership includes a team with dedication to each facility and service type, including Inpatient and Outpatient Clinical Services, Distribution Services Manager for PRMC and Parkview Hospital, an inpatient pharmacy supervisor, lead pharmacists in central and critical care/OR satellite, and lead technicians in central pharmacy. Parkview’s current ambulatory reach beyond anticoagulation includes pharmacists in Falls Clinic, Hypertension Clinic, Lipid Clinic, Movement Disorders Clinic, and Pediatric Complex Clinic.
