What You'll Do:

Based from your home office in the Metro Atlanta, GA region, as our Clinical Pharmacist your devotion to service is essential to meet the needs of our clients and residents. You’ll serve as liaison between the pharmacy and the living centers while providing leadership within medication management. Your passion for staying on the frontline of regulatory compliance, clinical guidelines and medication management will be put to excellent use as you collaborate with key stakeholders and the interdisciplinary team at the living center.

Your duties and the purpose of this role is to:

Provide leadership, knowledge, and expertise for all functions related to clinical pharmacy at the long-term care center level. Provide integration with an interdisciplinary team to improve patient and facility clinical outcomes associated with medication use. Most important is relaying your area of expertise by educating the appropriate center staff. As well you'll participate in continuous quality improvement and risk management activities that focus on medications or medication related systems that place patients and living center at the highest risk of negative outcomes.

Qualifications:

  • Registered Pharmacist Licensure in appropriate state(s)
  • Doctorate of Clinical Pharmacy (Pharm.D), preferred
  • Board Certification as Geriatric Pharmacist (BCPG) required or must be obtained within first three years of employment
  • Minimum three (3) years’ experience as a consultant pharmacist in Long-Term Care preferred
  • Proven ability for adherence to State and Federal regulations (CMS, DHHS, State Board of Pharmacy, etc…) as well as adherence to internal ethics, policies and procedures
  • Ability to travel up to 50% of the time with overnight stays
  • Must be dependable and capable of maintaining regular attendance
