Provides superior customer service to internal and external clients, customers and patients.
Accountable for the care and outcomes that his/her staff is providing.
Meets weekly with case management clinicians to review patient caseloads, assessing the quality of care, coordination of all disciplines involved and appropriate allocation of services.
Mentors new clinicians.
Performs joint visits with clinicians as a means of assessing clinician ability to manage patient care needs, leading to excellent patient outcomes and education of clinician in accurate assessment and documentation of assessment utilizing OASIS tool.
Analyzes patient data by clinician, noting trends and working with clinician to optimize patient outcomes.
Ensures fair and consistent treatment of all clinicians relative to caseloads, productivity, geography.
Collaborates with scheduling and Performance Improvement as team members to assist in managing his/her clinicians.
Facilitates effective weekly multi-disciplinary team meetings.
Operationalizes agency goals within the scope of responsibility of the Clinical Manager role.
Collaborates with telehealth coordinator and guides implementation for their team.
Plans and manages time effectively and productively.
Participates in task forces designed to improve patient outcomes.
Ensures that daily operations incorporate administrative and clinical performance improvement.
Ensures compliance with all federal, state and Joint Commission rules and regulations.

Graduate of an accredited school of nursing required.
Must have demonstrated competence and satisfactory performance with direct patient care in a community health or medical/surgical setting for a minimum of three years.
Must possess current RN licensure from the Massachusetts Board of Registration in Nursing.
Must possess current, valid driver's license.
Possesses working knowledge of third party reimbursement as well as regulatory requirements.
Working knowledge of computer and ability to learn home care software package.
Working knowledge of homecare reimbursement.
Working knowledge of Oasis documentation.
