Provides oversight, coordination, and insights for various Clinical Integration initiatives, including but not limited to Risk Score Accuracy, Utilization management and governance committees. Supports payer relationships and conducts analysis and maintains trending for populations in value based contracts. Strategizes and collaborates with key stakeholders in multiple markets to insure success in value based contracts, including quality initiatives and special projects. This role is vital in providing direct oversight of initiatives, subject matter expertise in population health program management as well as successfully supporting teams in large geographical regions.
II. ESSENTIAL JOB FUNCTIONS ESSENTIAL FUNCTIONS
-Responsible for population Patient Panel management strategy, tracking and identifying opportunities for improvement of the assigned value based contracts and cover larger populations of individuals over multiple states. Collaborates with other Clinical Integration -----Project Managers to compare trends and propose strategies that span all lives attributed in the assigned population health program.
-Acts as a liaison between Payer Leadership and BSMH key stakeholders and manages the overall relationship in coordination with the Managed Care Team. Includes payer-specific tracking, analysis, special projects and managing goals and expectations while coordinating with market based teams focused on meeting deliverables.
-Supports the Clinical Integration team in managing Affiliate success in value based contracts; includes analysis of data, providing insights, communicating with key stakeholders and appropriately communicating and elevating any potential issues or successes to leadership.
-Supports, collaborates and conducts analysis for Clinical Integration Teams and Operational leads within the assigned Regions to ensure success in higher quality patient care and meeting goals of value based contracts. May include organizing provider and/or patient outreach, provider and/or patient scheduling activities, bulk messaging or other special projects based upon an identified ministry initiative or quality or care management gap.
-Collects, tracks and analyzes practice demographics and population health internal and external trends. Monitors and elevates any potential issues, challenges or barriers.
-Supports development of positive professional relationships with key stakeholders. ---Maintains regular written, verbal, and electronic communication with Clinical Integration team members, Medical Directors and other key stakeholders on performance with the practice quality and utilization activities for each of their managed populations or special projects.
-Manages the overall orientation process, materials, scheduling and presentations for providers or key stakeholders coming into the assigned network for the clinical programs that guide success in value based contracts.
Collaborating and training Operations Leads or key stakeholders in data, insights, reports and opportunities for improvement on various Clinical Integration initiatives.
-Responsible for managing assigned vendor relations (ie: Emmi, Stanson, Patient Point) including tracking invoices, manages data and insights, monitors progress towards contracted goals and meets with key stakeholders.
EMPLOYMENT QUALIFICATIONS EDUCATION QUALIFICATIONS -
Required Minimum Education:
Master’s degree in a Healthcare related field Licensing/ Certification - If registration, certification or licensure is required, include the name of the accrediting or licensing body.
Not required. LPN/RN accredited by CCNE, ACEN, or CNEA. PMP, Lean Six Sigma or other Project Management/Lean Certification. Minimum Qualifications

5 years’ minimum in a healthcare setting with previous management and/or leadership experience.
