As a Registered Nurse Care Manager you will be responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. You will facilitate the progression and transition of care using established criteria in conjunction with the multidisciplinary team. You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
Qualifications
Required Education: Associate Degree in Nursing or Nursing Diploma
Preferred Education: Bachelor's Degree in Nursing
Required Experience: 2+ years' experience in Care Management or 3+ years' experience in clinical nursing
Preferred Experience: InterQual or MCG experience
Required License/Registration/Certification: Currently licensed as a Registered Nurse in the state(s) of practice according to the State Board of Nursing
Preferred License/Registration/Certification: ACM or CCM Certification
