The Care Management Program Manager leads the hospital's home and community-based care management programs focused on improving the health of Howard County residents. These multiple programs provide an array of services from chronic disease prevention, care coordination, community-based events/outreach and home visitation to help patients better manage their illness, improve their health and avoid preventable hospital encounters. The Program Manager is responsible for all aspects of managing programs, including planning, implementation, evaluation, related budget activities, monitoring performance, creating opportunities for continuous quality improvement and program growth/expansion. This position is responsible for hiring, orienting and supervising clinical and non-clinical program staff. The Program Manager serves as a link between care management staff and other disciplines/departments throughout the organization and within the community. The position reports to the Director of Population Health programs.


  1. Graduate of an accredited school of nursing.
  2. Master’s degree in Nursing, Public Health, Administration or related field required.
  3. BSN or Bachelor's degree in a health care related field and prior care management experience required.
  4. Minimum of five years of experience in public health or in a community setting. At least one of the years of experience must have been in an administrative, supervisory, consultative or teaching capacity.


  1. Current licensure as a registered nurse by the State of Maryland or licensure in a compact state that allows practice in Maryland.
  2. Must obtain CPR certification within sixty (60) days of hire and maintain thereafter.
