Job Summary:

Under general supervision, plans, conducts and may supervise assignments.  Manages and performs analysis, specification, design, development, implementation, operation and maintenance of Cardiovascular Database applications.  Responsible for coordinating care of selected patient populations throughout their hospital stay and participates in clinical process development to improve care.  Uses Clinical expertise and problem solving to collaborate with MD, patient, family and staff to achieve optimal clinical outcomes.  Will have working knowledge of PC and Cardiovascular Outcomes Database applications to include data collection, entry, review, analysis and reporting.  Functions in a project management/supervisory role and will assist in administrative responsibilities and strategic planning.

Establishes goals and procedures, analyze design, implement, evaluate and maintain Database system based upon end-user needs.

Patient facilitator for defined patient population for their hospital stay. Coordinates care with MD, patient, family and staff to assist with the physical and emotional needs of the patient and family.

Gathers and organizes information by observation, chart extraction, receiving and otherwise obtaining data/information from all relevant sources.

Analyzes data by identifying the underlying principles, reasons, or facts of information by breaking it down into separate parts, proposing and implementing solutions or alternative solutions.

Quality Improvement through tracking and facilitating compliance by medical staff with clinical practice guidelines. Identifies and plans strategies to effect change from outcomes data. Provides Interface with the PI system. Works in collaboration with MDs and appropriate healthcare providers for evidence based practice guidelines development, implementation and revision.

Remains current on healthcare practices and information technology trends.

Assists management with administrative responsibilities and clinical strategic planning.

Manages project implementations, installation, acceptance testing, system education and maintenance.


Education: BSN required or equivalent work experience. Master's degreepreferred.

Experience: 8+ years of experience in a clinical healthcare role.  Cardiovascular experiencerequired and Quality Improvement knowledge helpful.
