Manages the activities of the Care Transition Coordinators to ensure objectives are met and reimbursement regulations and accreditation standards are followed. Acts as a liaison between Agency's clinical operation and attending physicians to facilitate patient care. Develops and maintains community relations in a variety of settings. Serves as resource and provides education to patients, caregivers and others concerning Agency policies, procedures, services, etc.
Education Required: Bachelor's Degree in Business Administration, Healthcare Management, Nursing, or Liberal Arts.
Education Desired: Masters of Business Administration or other related health care area.
Experience Required: Three (3) years of sales experience in healthcare, community relations, or related field plus a minimum of one (1) year of sales management experience.
Experience Desired: One (1) year of management experience in home health care with an emphasis on physician relations activities.
