Assess, plan and provide all aspects of patient care, including diagnosis, treatments and consultations. Work both inpatient and outpatient performing independently or as part of a treatment team. Educate patients about preventative care and prescribed treatments. May also conduct physicals, order tests and serve as a patient's primary healthcare provider. May be required to prescribe medications. Assist hospitalist physician staff admit, care for and discharge patients in the hospital. Provide services to inpatients who have no primary care physician or patients whose primary care physician cannot visit the patient in the hospital under the supervision of the hospitalist physician. Consult with physician regarding any task, procedure, or diagnostic problem, deemed necessary. Comprehensive care including care coordination and self-management building skills to promote better outcomes for patients. As part of the team, there will be an emphasis on disease prevention and wellness, as well as being included in the treatment of acute and chronic diseases. This position will help provide care management and support to the patient/family/caregiver prior to treatment, during treatment and following treatment with transitional care and coordination across all elements of the broader health care system. This would include specialty care, hospitalization/ER, home health care, community services and support, test tracking and follow-up, referral tracking and follow-up, and care coordination transitions from one health care facility to another facility or to home. May require assisting in ER, which they will work in collaboration with a physician to assess the physical needs of patients admitted to ER. Initiate or assist in patient care in an effective and responsive manner, as each situation requires. Triage each emergency appropriately and involve other resources as appropriate. Administer and perform emergent care in accordance with the applicable patient population type, which may range from newborn to geriatrics. Follow emergency policies, procedures and protocols in an effective manner. 7. Perform and maintain documentation. Document accurate and ongoing assessment of patient status that reflect interventions, patient responses, patient teaching and status of outcomes. 8. Communicates effectively. Communicate and collaborate effectively with other health team members regarding patient condition, plan of care, patient satisfaction needs and recommendations for meeting identified outcomes.
