MAJOR DUTIES AND RESPONSIBILITIES:
*A. CLINICAL ACTIVITIES:
1. Assumes responsibility for the delivery of pharmaceutical care and medication management to the patients that are referred to the Anticoagulation Management Program and Medication Therapy Disease Management Program. Coordinates the use of human, material, and informational resources such that timely, efficient, cost effective and appropriate care is consistently rendered.
2. Demonstrates proficiency in medication and disease management. Receives and interprets physicians' referral orders and clarifies or corrects orders with the physician. Reviews patient medication profiles to guard against drug sensitivities, contraindications, duplications, and major drug interactions.
3. Gathers appropriate clinical and laboratory data necessary in order to make appropriate decisions on the management of the patient’s anticoagulation therapy, medication therapy and related diseases/conditions. Orders appropriate laboratory monitoring in order to insure appropriate patient follow-up information is available.
4. Assesses patient’s medication therapy regimen for appropriateness, effectiveness, and potential for adverse events. Make appropriate changes to regimen which will maximize effectiveness and patient satisfaction, while working to minimize potential for both acute and chronic complications
5. Reviews all medication orders for prescribing, formulary, and therapeutic issues. Suggests appropriate formulary alternative(s) based on the patient's clinical parameters.
6. Consults and advises members of the medical staff on matters relating to specific drug therapy problems and monitors and acts on patient laboratory results. Provides in-depth drug information and other informational materials to members of the medical, nursing and otherhealthcare professionals. Provides patient discharge medication consultations based on department standards. Calculates drug dosages based on pharmacokinetic parameters.
7. Provides patient with appropriate educational materials and information in order help them understand and actively participate in their care.
8. Prepares articles for the drug information newsletter and other Geisinger publications and serves as editor as requested. Prepares drug reviews for Enterprise Pharmacy Formulary Subcommittee.
9. Provides appropriate documentation of each encounter in the electronic health record and conveys that information to the referring physician/next provider of care as appropriate.
*B. EDUCATIONAL / TEACHING ACTIVITIES:
1. Attends department educational programs. Maintains a current knowledge of technical and clinical practice changes affecting their scope of practice. Presents staff development programs for the Enterprise Pharmacy Department.
2. Teaches pharmacy interns, pharmacy residents, students, pharmacy technicians and pharmacists and trains new pharmacists, regarding information in all appropriate areas of pharmacy practice, policies, and procedures.
3. Presents posters/papers at professional meetings at national, state and/or local level; publishes articles in medical and pharmacy journals.
4. Provides inservices to otherhealthcare providers when requested and/or when necessary.
5. Demonstrates competency in all practicing disease states by passing annual competency exams
6. Assists the Director, Assistant Director and Regional Coordinator in implementation of new MTDM programs including building and organization of the new program, education and certification of all staff members
*C. ADMINISTRATIVE ACTIVITIES:
1. Demonstrates ability in operational area with respect to time management, supervision of staff, problem identification and resolution, and decision making.
2. Coordinates and supervises work-place activities and resources; promotes professional competence; assures that services rendered and work performed are in accordance with departmental standards, state/federal regulations, and billed appropriately. Advises, assists, and supervises the work of pharmacy technicians and pharmacy interns; assumes final responsibility for accuracy of their work.
3. Works with medical and/or nursing personnel to resolve problems involving pharmacy MTDM services. Coordinates activities with other pharmacy personnel to accomplish pharmacy and hospital objectives. Works with facility and on-site staff/administration where necessary to resolve issues, incorporate MTDM clinic services seamlessly into site workflow, and to continually demonstrate the value of pharmacy MTM services
4. Assists with the review and revision of departmental policies and procedures, as requested. Applies departmental policies and procedures to complex situations and identifies areas for which policies and procedures need to be developed, improved or revised.
5. Assumes responsibility for operation/supervision of the Anticoagulation Clinic/Medication Therapy Disease Management Clinics when assigned. Assists Director, Assistant Directors and Regional Coordinator of Clinic Pharmacy Programs in obtaining appropriate up-to-date, signed collaborative practice agreements for the patient populations responsible for.
6. Participates in various local, state, and national organizations. Participates on various departmental and hospital committees and teams as assigned and serves as chairman or secretary as requested.
D. MISCELLANEOUS ACTIVITIES:
1. Represents the Enterprise Pharmacy Department in a credible and professional manner. Maintains a professional attitude and appearance when practicing in an inpatient or outpatient setting, a tie for men and conservative street attire for both men and women is the standard.
2. Assists patients with drug-related problems.
3. Performs other duties as required or assigned for emergency or other operational reasons for which the employee is qualified to perform.
4. Demonstrates C.I.CARE in all aspects of daily activities
