Position is responsible for providing leadership to the weekday evening Support Services Staff and oversight of all aspects of services provided to patients and caregivers during the weekend.

DUTIES AND RESPONSIBILITIES
Functions as a role model in area of clinical specialty, demonstrating competency in clinical
judgement, selected technical skills and appropriate knowledge base to lead and guide staff. Demonstrates clinical expertise in the execution of the nursing process and directs staff in its application to promote safe, effective and appropriate care.
Supervises, evaluates and mentors staff assigned to Triage, Pharmacy Support and
On-call, inclusive of creating and overseeing a plan of professional improvement for each team of workers member to ensure updated credentials and compliance with certification and licensure requirements. Prepares and provides workforce critiques in a timely manner. Establishes and oversees completion of character overall performance development plans as essential.
Serves as a resource to staff, patients and families, ensuring that patient problems are addressed and resolved in a timely manner.
Evaluates the quality of patient care through direct staff observation and review of nursing documentation, review of medical records and patient feedback.
Ensures that staff understands and exhibits the hospice philosophy of palliative care through education and role modeling.
Conducts supervisory visits for the staff.
Liaisons with the Director of Human Resources to ensure that all staff needs and issues are addressed in a timely manner. Be familiar with and administer the Progressive Discipline Policy as needed.
Ensures that staff is familiar with and adheres to Hospice of the Chesapeake policies and procedures.
Patient careresponsibilities include participating in Pediatrics On Call, and GIP visits.
Supports and participates in process improvement initiatives as directed by Clinical Director.

-Graduate of an accredited nursing school required; bachelor’s degree in nursing strongly preferred. Possess a current Maryland nursing (RN) license.
-Minimum three years of nursing experience in a medical/surgical or home care setting/previous hospice experience strongly preferred.
-Minimum three years of supervisory experiencerequired.
Excellent oral, written and interpersonal communication skills.
Demonstrated emotional maturity and ability to cope and work effectively in stressful
Demonstrated ability to work independently with minimal supervision.
Knowledge of Medicare, Medicaid, private insurance and other community programs and
Demonstrated ability to work effectively on an inter-disciplinary team.
Excellent customer service philosophy and skills required.
Demonstrated ability to maintain confidentiality of information.
Demonstrated organizational and time management skills.
Basic computer skills, to include but not limited to Microsoft Word and Outlook, required.
Demonstrated understanding of and concurrence with the hospice philosophy of palliative care.
Must maintain a valid driver’s license with a good driving record.
Must be able to safely move and lift 50 pounds.
Compliant with health screening and vaccination requirements of organization and position, including TB screening and Influenza vaccination annually.
