The Clinical Manager provides direct oversight of all patient care in accordance with agency policies. This oversight includes patient and personnel assignments, coordination of care, coordination of referrals, assessment of patient needs and ensuring the plan of care is developed, implemented and updated. The Clinical Supervisor provides direct supervision to clinical personnel, and may assume responsibility for the department in the absence of the Director.
Minimum Education Training & Experience (includes licenses or certifications):
Education:RN - Bachelor's Science in Nursing, Master's preferred.
Licensure/Certifications: Licensure in both Washington and Oregon, or the ability to obtain upon hire. OASIS certified or ability to obtain OASIS certification within 1 year of hire. Current Health Care Provider CPR.
- Minimum of three years progressively responsible clinical home care experience, with five years required in home healthcare.
- Proven ability to work within an interdisciplinary setting.
- Knowledge of state and federal home health agency regulations.
