The Health Resource Manager Supervisor is responsible for providing case management services for Southwestern Vermont Medical Center patients and Centers for Living and Rehabilitation residents.

Requirements: Graduation from an accredited school of nursing; Bachelor's of Science in Nursing or in active matriculation required.

Five years of clinical nursing experience required. Prior experience in utilization review, discharge planning and/or case management required.

Ability to maintain a valid Vermont State Registered Nursing license. Case Management Certification with 2 years of hire is required.
