Provides case management/therapy for patients and families to enhance their psychosocial functioning. Performs duties and responsibilities in a manner consistent with mission, values and mercy service standards.

- Master's degreerequired in Social Work, Counseling or other human-service related field.
- Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) required.
- Minimum 2-3 years Behavioral Health experience.
