GENERAL SUMMARY OF DUTIES – The pharmacist processes medication orders; supervises pharmacy technician activities; oversees the distributions and procurement of biologicals and pharmaceuticals, including controlled substances; participates in the delivery of pharmaceutical care; and performs other duties as assigned.  Regular attendance is an essential function of this job.

  • Maintains competency in unique aspects of drug therapy for all patients served
  • Provides pharmaceutical care by collaborating with physicians,nurses andother members of thehealthcareteam in:
    • Reviewing patient medication history
    • Educating patients on drug therapy
    • Coordinate in the development of therapeutic plans to attain desired outcomes
    • Determining and recommending alternatives for achieving outcomes
    • Monitoring patient outcomes
    • Documenting medication related issues in patient chart
    • Interpreting laboratory data
    • Reviewing patient medication profiles
    • Monitoring pharmacokinetics of drug therapy
    • Monitoring potential adverse drug-drug and drug-food interactions
  • Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication and drug interaction
  • Evaluates medication orders for appropriateness and compatibility with disease state and existing orders. Makes dosing recommendations based upon pharmacokinetics and other clinical parameters
  • Concurrently monitors use of drugs for compliance with prescribing restrictions/criteria, guidelines and protocols
  • Precepts pharmacy students
  • Conducts drug usage evaluations as needed
  • Detects, monitors and reports adverse drug reactions and medication errors
  • Discusses medication order clarifications with the prescriber, documents clarification in patient record and communicates clarification/changes to others involved in provision of care
  • Promotes the use of the formulary by converting non-formulary orders to formulary products either through automatic substitution protocols, therapeutic substitution protocols or by contacting the prescriber
  • Documents clinical interventions in the appropriate forum
  • Insures proper technique and accurate preparation of all pharmaceutical products including knowledge and adherence to USP<797> Standards and hospital Policy regarding sterile compounding.
  • Oversees and verifies technician functions.
  • Participates in training of new staff and ongoing training of technicians
  • Insures timely and accurate dispensing of drugs and solutions from the pharmacy area
  • Provides drug information to healthcare professionals and patients
  • Takes active role in quality improvement activities
  • Is competent and can work with and knows how to resolve issues regarding the hospital’s point of care pharmacy automation system (Pyxis)
  • Is proficient with the pharmacy’s IT (Meditech) System
  • Other duties as assigned.


  • Demonstrated proficiency in written and verbal communication
  • Ability to work independently
  • Flexibility and ability to prioritize tasks
  • Demonstrated critical thinking skills
  • Ability to interact in a professional manner with patients, medical staff and other staff
  • Has a strong working knowledge of computers and Microsoft Office


  • Graduate of ACPE accredited college of pharmacy


  • One year prior health-system or long term care pharmacy experiencepreferred.


  • Licensed or eligible for licensure in Iowa as a Registered Pharmacist
