Responsible for providing quality education, follow-up and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.
Education - Bachelor's Degree or equivalent in Nursing
Experience - 5 years of experience
Licenses - Registered Nurse (RN) Valid State of Michigan RN license
Basic Life Support (BLS) must obtain within 60 days of hire
Breast Navigator within 1 year of hire
Oncology Certified RN (OCN)
