Program Highlights: The AHCaH program employs remote patient monitoring, advanced technology, and interdisciplinary collaboration to provide exceptional care to eligible patients. Target Population & Conditions Treated: Med/Surgical level of care Congestive Heart Failure (CHF) exacerbation COPD exacerbation Urinary Tract Infections (UTIs) Other appropriate conditions Your Responsibilities as an Advanced Practitioner: As a key member of the AHCaH team, you'll work closely with patients and collaborate with a dynamic interdisciplinary care team under physician guidance. Your main duties include: Conducting in-person and remote patient assessments Developing and implementing treatment plans Providing continuous monitoring and coordinating with field staff Team Composition: Advanced Practitioner: Primary provider handling patient care RN Coordinator: Responsible for care coordination and program oversight Field Staff: Mobile paramedics and RNs delivering in-home care Physician: Supervising direct care and interdisciplinary team Location: This position is based at the Geisinger Healthplex CenterPoint office in Pittston, PA, with occasional travel required to Geisinger Wyoming Valley, Geisinger South Wilkes-Barre, and Geisinger Community Medical Center in Scranton.
