The Case Manager facilitates the coordination of the patients' care from pre-admission through hospitalization, to post-discharge follow-up, in collaboration with the healthcare team, to best meet the patients' physical, emotional, and spiritual needs.

Creates budgets for proposed programs determining direct funding, "in kind" value and contributions from other sources. Determines appropriate grants and other funding sources to ensure continuation of workforce programs.
This position oversees the implementation of a program consistent with the mission of MCHLB. The incumbent assists with the preparation of the strategic business plan, financial Performa, market conditions and plan and customer service plan.
This position is accountable at the strategic level for planning, developing new services, policy formation, human resource management and organization, financial performance, patient care services operations, senior medical and management staff relations, and participation in LBMMC/ MCH strategic planning as a member of the LBMMC/MCH senior management team.
Takes ownership of their accounts and eliminates barriers to ensure timely and correct payment including involvement with payors upper management, when necessary. Must have working knowledge of managed medical including other complex payment methodologies to ensure prompt and accurate reimbursement.
