The Case Manager facilitates the coordination of the patients' care from pre-admission through hospitalization, to post-discharge follow-up, in collaboration with the healthcare team, to best meet the patients' physical, emotional, and spiritual needs.

Creates budgets for proposed programs determining direct funding, "in kind" value and contributions from other sources. Determines appropriate grants and other funding sources to ensure continuation of workforce programs.
This position oversees the implementation of a program consistent with the mission of MCHLB. The incumbent assists with the preparation of the strategic business plan, financial Performa, market conditions and plan and customer service plan.
