The Manager, Care Management, is responsible for the oversight, management and optimization of patient centered care coordination activities. The manager oversees a team of care managers and is responsible for the day-to-day operations of the care management program.
The IT Systems Engineer is responsible for Windows System administration, installation of new systems and technologies, and ongoing operations and maintenance for various networks and computing environments.
Assist with solution testing and ensure that the solution delivered meets the business need as defined in the business requirements; follow up with business owners to clarify business requirements and process flows.
Participates in collaborative meetings between care center staff/providers and care management across all markets, leads clinical operations oversight meetings and maintains agenda and written meeting minutes.
Communicate closely with patients' other healthcare providers so that primary and specialty care is well coordinated; build a successful home visit practice that carries an excellent reputation among area patients and healthcare professionals.
Design, develop, and unit test new database code to support requirements, using database triggers, stored procedures, and views; work closely with business and IT stakeholders to define problems and design large-scale, enterprise class solutions.
