The Social Worker is responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.
In this role, the selected candidate applies advanced social service knowledge to develop Individualized Services Plan (ISP) to establish the appropriate services to be provided for each child in placement and to assure compliance with state regulations.
The Social Worker is responsible for clinical evaluation and management of patients in a home setting, as assigned by the Primary Care Physician, Medical Director, Hospitalist or Care Manager. Use your natural gift of education and persuasion to help patients and their families in an outpatient setting deal with a variety of complex conditions.
In this role, you will be responsible for the interdisciplinary team whose primary function is to provide diagnostic evaluations of the psychosocial problems of the patient/family unit, defining social service goals for alleviating these problems, and helping to provide both the counseling and casework needed to meet these goals.
In this role, the selected candidate will provide medical social work services as ordered/referred by the attending physician to Hospice patients in their home, according to Hospice policies and procedures and state and federal regulations governing Hospice care.
