In this role, you will be completing a comprehensive psychosocial assessment and collaborating with the ambulatory care team regarding the plan of care. The medical social worker will assist patients who are participating in the end of life options act. The medical social worker will also assist with completion of end-of-life care documents.
The Medical Social Worker provides appropriate patient/family counseling for issues related to life changes, negative family dynamics, community resources and assistance with application for alternative funding, income related sources and initiates the patient outcome reporting process.
