Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs. Implements and maintains established hospice policies and procedures as well as standards of care and regulatory requirements.
In this role you will have proficient knowledge and understanding of utilization management, case management, healthcare finances, alternative care options, goals and outcomes and regulatory and professional implications.
Interpersonal skills necessary to interact effectively with members of the interdisciplinary team, including physicians and external health care professionals, to achieve desired clinical, service and financial outcomes.
The CM will coordinate with the health care team and the patient/family to address payer issues that impact care, and to develop a transition plan of care that includes a safe discharge plan and plan for follow up with care providers post discharge.
This position coordinates patients care with an interdisciplinary team; screens and coordinates authorization of requests for capitated services for all assigned managed care where the hospital has a financial responsibility.
Performs evaluation in multiple environments including process and relationships, health care management, community resource and support, service delivery, psychosocial intervention and rehabilitation.
