The Social Worker is responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.
In this role, you will conducts psychosocial assessments, and provides services to patients and families experiencing major trauma, chronic illness, child abuse and neglect, ingestions, financial concerns, discharge planning and complex family situations.
In this role, the selected candidate applies advanced social service knowledge to develop Individualized Services Plan (ISP) to establish the appropriate services to be provided for each child in placement and to assure compliance with state regulations.

In this role, you will provide established outcomes driven psycho-educational counseling methods aimed at improving areas of need, treatment adherence, vocational/educational rehabilitation and/or quality of life.
In this role, you will demonstrate the knowledge and ability to care for age specific needs of the population served as measured by completion of mandatory training and direct observation by supervisor.
