In this role, you will establish a collaborative relationship with the client (plan participant), family, physician(s), and other providers to determine medical history and current status to assess the appropriate level of care and options for alternative care.
In this role, you will assesses and evaluate patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and making necessary revisions to the plan.
In this role, you will educate your patients on how to do their own dialysis from their homes, and you take responsibility for the complete ongoing care of each patient to ensure their treatment stays on track.
In this role, you will complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs. He / She will provide direct nursing care to residents, in compliance with the Nurse Practice Act, and federal, state and local laws.
In this role, you must have have a current RN license in the State of Ohio, 1-2 years home care or community nursing experience, demonstrated excellence in nursing assessment, home care case management, communication and problem-solving skills.
As a RN Case Manager, your ability to manage client care with specific knowledge and experience in bedside care, symptom management, crisis intervention and family intervention are feature assets for this high-profile nursing position.
The Case Manager possesses strong clinical skills and a knowledge base in a specialized area, and utilizes leadership skills in coordinating and/or providing home care services for a group of patients within that specialty area.
