This position will be responsible to participate and support the service line directors and interdisciplinary team members in the overall planning, development, coordination, implementation and management of a comprehensive regionally-focused strategy.
The individual in this position is responsible for the psychosocial assessment of patients, for post-hospital discharge needs including home care, nursing home placement, durable medical equipment, financial assistance, counseling, and other community resources.

In this role, you will be responsible for the overall operations of the Washington branches, ensuring that all in-home support client services are delivered in a safe, efficient and effective manner, in accordance with established quality standards and applicable state and federal regulations.
In this role, the selected candidate will provide support in a group setting appropriate to the need of the members recovery goals; develops outcome measures for these groups; determines the appropriate composition and creates activities to facilitate recovery goals.

Occasionally performs moderately complex assignments that require initiative and independent judgment in the selection and use of established principles and procedures. In this role, the selected candidate must have a proven experience in project delivery processes that include conceptual through detailed design, process optimization, and opinions of probable cost, as well as constructability and quality assurance reviews.
