The RN Case Manager is a member of the Clinical Resource Management Department working collaboratively with the multidisciplinary healthcare team to ensure the appropriate allocation of hospital resources while maintaining the quality of care.
The RN Case Manager will manage each patients care efficiently and effectively to achieve positive patient outcomes, and be responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPN, HHA) who will be directed / supervised by the RN.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
Serves in an expanded nursing role to collaborate with Skilled Nursing Facilities, and the Medical Home team to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services.
In this role, you will collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and /or health needs through communication and available resources to promote optimal, cost-effective outcomes.
Manage in a case management system (EMR) using ongoing interaction with patients, physicians, and other health providers to meet designated clinical, operational, and financial outcomes for aggregate patient populations.
The RN Case Management utilizes their skills to collaborative assess, plan, facilitate care coordination, and advocate for options and services to meet an individuals and familys comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
