The Primary Care Physician will be responsible for outpatient work, but will have some opportunities for hospital work and teaching. This position is accompanied by a competitive compensation package with significant benefits.
The APP will collaborate with designated physicians and healthcare team members to provide medical care to an assigned caseload of inpatients as part of the Renal Service inpatient consultation team. The patient mix includes patients with acute and chronic renal disease, complex electrolyte disorders, and patients on dialysis.
In this role, the selected candidate will p rovide assistance to initial clinical reviewers as needed to discuss cases and problems; review all imaging cases in which clinical determinations cannot be made by initial clinical review.
Working with a care team that includes Leukemia Attending Physicians, Hematology/Oncology Fellows, Clinical Pharmacists, Care Coordinators, Social Workers, and a dedicated Oncology Nursing Staff, you will help develop, coordinate, and deliver the daily care plan.

Performs and / or participates in rounds of hospitalized patients, including physical assessment, interpretation of diagnostic studies, interaction with nursing staff and documentation in medical record.
In collaboration with the physician, provides and coordinates primary preventive care, emphasizing functional independence and treating a members physical, social and psychological needs. Insures development of plan of care and access to services in accordance with the goals and objectives by communicating and collaborating with the interdisciplinary team including the nursing home administrative staff.
