Our Social Workers play an essential role in providing a positive experience for patients and their families by providing case and group work services during the hospital stay. The Social Worker acts as an advocate on behalf of the patients to ensure maximum access to quality healthcare.
The selected candidate will participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
In this role, the candidate will be responsible for providing bio-psychosocial mental health assessments and crisis intervention services to patients of all ages including their families/support system and recommending the treatment plan to physician.
In this role, you will be responsible for intervening with patients and families, by applying critical thinking skills to monitor psychosocial status and issues that impact the clinical progression and transition/discharge plan for patients, including the facilitation of patient and family communication, coping and decision making Works as a part of a multidisciplinary team.
