The Social Worker supports the business development activities of the organization; as a member of the hospice interdisciplinary team, provide social service counseling and guidance to facilitate the maximum function and coping capacity of each patient and family member while following all hospice policies and procedures, in accordance with the interdisciplinary plan of care.
In this role, you will implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
In this role, you will provide social work services to Cook Children's Health Plan (CCHP) members across the health care continuum. Health Plan Social Workers collaborate with Health Plan Case Managers to develop and implement an individualized plan of care that addresses the medical, psychosocial and educational needs of the member.
In this position, you will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with current federal, state and local standards, guidelines, and regulations, our established policies and procedures.
In this role, you will be responsible to provide patients and family members with the education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the state board of social worker examiner scope of practice.
