In this position, you will perform ongoing essential Case Management activities of assessment, problem identification, planning, implementation, coordination, monitoring, and evaluation of case managed members.
The Case Manager identifies, evaluates, and provides management of services for patients with complex, catastrophic, long term illness or injury, targeted diseases, as well as those patients receiving out of network services.
The Case Management Manager supervises and leads the Utilization/Case Management Department, including planning, organizing, and coordinating the activities of the Case Management program. Demonstrates mastery of the US Family Health Plan Case Management program, managed care environment, TriCare, DoD, Utilization Management and Case Management standards, managed care software and USFHP Policies and Procedures.
